Permit Fee/Fine
Food Establishment Permits are due in June each year. To better serve our community we have added an online payment feature for your convenience.
Please add your invoice number to the notes portion when making an online payment.
Fines are due within 30 days of receipt.
NOTE: All Food Establishment Applications must be signed, dated, and returned to the Health Department before Permit is released.
Out of State Birth/Death Certificates

NOTE: All out of state applications must be received signed, dated, and notarized.
CPR/First Aid Class
ALL CPR/First Aid classes MUST be scheduled in advance!
Please call (573)346-5479 to schedule.
NOTE: CPR class payments are non-refundable and are good for reservations in one class only. You must keep your reserved class seat in the class you specify in order to attend. No refunds for no shows, no exceptions.
